We are pleased to welcome you to Wyoming Brain and Spine Associates. Our doctors, physician assistants, and staff members are dedicated to your health and well being and we appreciate the opportunity to participate in your medical care.

To make your visit to our office as pleasant and efficient as can be, we have enclosed some forms to assist you in providing us with necessary information. This information will be used to ensure that we comply with your insurance carrier’s requirements and will also assist the doctor in providing your care. Enclosed you will find your Registration form, History and Physical form, Financial Policy Agreement, Authorization for Disclosure of Health Information, and a map directing you to our office. Please fill out all information on the forms, sign where indicated, and bring them with you when you come for your appointment.

We ask that you arrive 30 minutes prior to your scheduled appointment time to allow for proper preparation prior to being seen by our providers. Please bring your insurance cards with you to your visit. We participate with Medicare and Medicaid. If you are not a member of one of these programs, we request that you pay for office services at the time of your visit. For your convenience, we accept MasterCard and Visa.

If for any reason you need to change or cancel your appointment, please phone us as soon as possible so that we may accommodate you and/or other patients accordingly.
